JCPS Second Vaccination Dose Update


JCPS SECOND VACCINATION DOSE UPDATES
 
The Health Department has updated information for those individuals scheduling second doses of the COVID-19 vaccine. Here are important details you need to know:
 
THE DATE ON YOUR CDC VACCINE CARD IS NOT YOUR CONFIRMED APPOINTMENT DATE FOR THE SECOND DOSE OF THE COVID-19 VACCINE.
 
For JCPS employees who received their first dose before 2/1/2021:
 
LouVaxSupport@louisvilleky.gov will email you approximately one week before you’re eligible for a second dose to detail the process for scheduling your second dose appointment. 
 
No later than on the 23rd day after your first dose, you will receive an email about scheduling your second dose appointment. This email will come from noreply@salesforce.com and it will include “on behalf of LouVax Community <louhealthcovidvaccines@louisvilleky.gov>”
 
The email will be sent to the email address that was provided to us when you signed up for your first dose. 
 
The email will contain instructions on how to complete the necessary forms (personal information, health screening, and the vaccine consent) prior to showing up for your appointment. Once you complete the required paperwork, you will be able to schedule your second dose.
 
NOTE: The CDC advises to receive your second dose as close to the recommended interval as possible, and states that if you are unable to adhere to this timeframe, you may schedule to receive your second dose between 24 and 42 after your first dose. Note that the vaccine card is not confirmation of a scheduled appointment; you must schedule your second dose appointment.
 
For JCPS employees who received their first dose on 2/1/2021 or after:
 
If you have not already, you will receive an email and/or a text message from the Health Department that contains information about your appointment to receive the second dose of the vaccine. If you need to reschedule this appointment, log into your account via the link provided in the message or log into your account by visiting https://louvax.my.salesforce.com/. If you need further assistance, please email LouVaxSupport@louisvilleky.gov or call 502-912-8598.
 
NOTE: If you cannot return on the exact scheduled appointment date, it is acceptable to adjust the schedule by a few days. The CDC advises to receive your second dose as close to the recommended interval as possible, and states that if you are unable to adhere to this timeframe, you may schedule to receive your second dose between 24 and 42 days after your first dose.
 
TRANSPORTATION RESOURCES
 
The district will provide transportation to Broadbent Arena again for anyone who needs it.
